Thank you for scheduling an appointment with ENT for Children. We are here for you every step of the way. In order to minimize office wait times it is important that you follow the process below for scheduling and preparing for your appointment.

New Patient Steps
- Click here to request an appointment and our staff will be in contact with you.
- The safety and health of our patients and families is our top priority at ENT for Children. For that reason, we have transitioned to a zero contact intake. Once you have made an appointment, you will receive a link from our office that contains all the required forms. Please complete the pre-registration prior to coming to your appointment. You may call the office with any questions and we will be happy to help you.
- Arrive at your appointment with the following in hand:
- Insurance card.
- Photo ID.
IMPORTANT NOTICE FOR NEW PATIENTS
It is a legal requirement for you to bring the two items as requested above for us to be able to treat your child for the first time. We are highlighting this so strongly because we need you to understand the importance of bringing these documents in hand.
If you do NOT bring your Insurance Card and your Photo ID you will probably need to reschedule your appointment.
ENT for Children Telemedicine
We are pleased to now offer Telemedicine patient visits for your child’s ear, nose and throat problem. Make a Telemedicine appointment by calling our office staff. Please have your insurance card and credit card available for payment of any copays, charges.
